Position Title: Outreach Coordinator

Classification:  Full-time, exempt position

Reports to: Navigation Director

Location: Cuyahoga/Huron/Lorain & Cuyahoga/Stark/Summit Ohio-based positions

 

Medworks plays an integral role in helping uninsured and underinsured people in Northeast Ohio gain access to health care. Medworks is a Cleveland-based non-profit organization providing free health care via mobile clinics to anyone struggling to afford care, no qualification questions asked. Medworks also enrolls people in health insurance, explains benefits and community resources, and connects people to medical homes for ongoing care so they may lead healthier lives. Medworks partners with local hospitals, FQHCs, community partners, and medical and non-medical volunteers to make this possible.

Position Summary

The Outreach Coordinator serves as the primary liaison for individuals and families who need health care services. They facilitate the initial introduction to the organization’s services and maintain ongoing contact with each assigned client. The Outreach Coordinator will execute outreach and education campaigns, provide facilitated enrollment services, and coordinate referrals between relevant entities, working individually and as part of a team.

 

Primary Responsibilities

  • Develop rapport and relationships that foster trust to ensure clients feel comfortable and safe
  • Provide face-to-face visits and make outreach calls to the client based on their needs
  • The ability to communicate a fundamental understanding of financial assistance programs, Medicaid, and ACA Marketplace enrollment qualifications
  • Provide courteous, professional, and confidential assistance in a manner that is culturally appropriate to all clients seeking health benefits
  • Conduct marketing and outreach within local communities about health insurance options
  • Help local community members access health care by enrolling them in the Health Insurance Marketplace, Ohio Medicaid, and/or the Access Plan via Medworks clinics, community health fairs, educational events, and utilizing the Medworks helpline
  • Work with individuals to help them identify health care facilities that they can access for ongoing health needs
  • Determine eligibility and complete an application for Prevent Blindness Ohio to help clients receive free eye exams and glasses
  • Breaking down complicated health care information so it is easily understandable to our clients
  • Presenting a warm, open presence for clients during every interaction
  • Documenting clients’ preferences, questions, and issues while respecting privacy laws for patient protection (HIPAA)
  • Maintain accountability, reliability and dependability when performing all assigned job duties
  • Work independently, travelling throughout the assigned territory to provide services and meet with scheduled appointments in various locations
  • Ensure that follow-up is completed to capture client referrals and outcomes

 

Requirements & Qualifications

  • Bachelor’s Degree preferred, or 4+ years relevant work experience
  • Knowledge of the Access Plan eligibility and healthcare reform changes
  • Familiarity of payment systems and resources available to assist low-income/uninsured clients
  • Must be organized, detail oriented and can work effectively under pressure in both a team setting and individually
  • Demonstrated commitment to the field and a passion for serving under-served and under-resourced individuals, families and/or communities; particularly people living in poverty
  • Successful completion of the Department of Health & Human Services (HHS) Certification for Certified Application Counselors/Navigators
  • Ability to pass a background check and drug test
  • Able to handle confidential materials professionally with discretion
  • Willing to be flexible and adapt to evolving team needs
  • Proficiency with Microsoft Office (Word, Excel, and PowerPoint)
  • Bilingual/Spanish-speaking a plus
  • Experience working with nonprofit organizations or community groups strongly preferred
  • Experience working in non-profit organization or community clinic preferred
  • Clear and effective verbal and written communication skills
  • Ability to work with people from diverse backgrounds
  • Valid Driver License with safe driving record
  • Passion for the Medworks’ mission

Compensation & Benefits

The salary will be commensurate with experience and qualifications. Annual potential for salary increases is based on organizational and individual performance. Benefits include competitive health, dental, vision insurance, 401(k) plan, PTO, and remote work flexibility.
